Job summary

Oak Hill in Andover, CT is seeking a Direct Support Professional for evening shifts. This full-time DSP helps individuals with intellectual and developmental disabilities live with independence, dignity, and community involvement.

You will assist with daily living, monitor health, administer medications per plan, transport to appointments, provide mealtime support, document in the EHR, and ensure safety and compliance. Paid training and growth opportunities are provided.

Qualifications

• Must be at least 18 years of age.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment.
• Commitment to continuity of care, including occasional shift extensions for coverage.

Responsibilities

• Support daily living activities to help individuals live healthy, meaningful lives.
• Provide personal care, grooming, bathing, dressing, and hygiene support.
• Assist with continence care and encourage independence where possible.
• Monitor health information, document vitals when required, and follow care plans.
• Administer medications per training and track records in the EHR.
• Transport individuals to appointments and community activities using agency vehicles.
• Assist with meals and nutrition according to dietary needs and personal preferences.
• Support mobility and transfers using approved techniques and equipment.
• Maintain documentation of daily activities, health info, incidents, and med administration.
