Job summary

Oak Hill is seeking a Substitute Direct Support Professional (DSP) in Bolton, CT to support individuals with disabilities in a residential setting. The role involves assisting with daily living, personal care, health monitoring, and community participation, with a per-diem schedule and a starting pay of $20.39/hour (differentials based on shift).

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment per Oak Hill policies.
• Commitment to continuity of care, including occasional stay beyond scheduled shift.

Responsibilities

• Support individuals in daily living activities.
• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor and document health-related information and vital signs when required.
• Administer medications per training and maintain medication records.
• Follow health care plans and support overall well-being.
• Transport individuals to appointments and community activities.
• Prepare meals and snacks per menus and dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and medication administration in the EHR.
• Follow Oak Hill, DDS, DPH, HIPAA, and regulatory requirements.
• Respond to emergencies and implement behavioral support strategies.
