Job summary

Oak Hill in Mansfield, CT is seeking a Direct Support Professional (Substitute) to assist individuals with intellectual and developmental disabilities in a residential setting. You will provide person-centered support, promote independence, monitor health, assist with daily living activities, meals, mobility, and documentation in the electronic health record (EHR).

We welcome former DSP/CNA or those new to the field; paid training and certifications are offered.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license with the ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment.

Responsibilities

• Support daily living activities.
• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor health information and vital signs when required.
• Administer medications and maintain records as trained.
• Follow health care plans and support overall well-being.
• Transport individuals to appointments and activities.
• Prepare meals and snacks according to dietary needs.
• Assist with mobility and transfers using approved methods.
• Document daily activities and health information in the EHR.
• Follow Oak Hill and regulatory requirements and safety protocols.
• Respond to emergencies and implement behavioral support strategies.
