Job summary

Oak Hill in Bolton, CT is seeking a Direct Support Professional (Substitute) to provide person-centered support in a residential setting, empowering individuals with intellectual and developmental disabilities to live with greater independence. Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to pass background checks, fingerprinting, and health screenings.
• Ability to complete required trainings and certifications.
• Commitment to a safe, healthy, professional work environment and continuity of care.

Responsibilities

• Support individuals in daily living activities and personal care.
• Monitor health information and document vital signs when required.
• Administer medications per training and nursing directives.
• Transport individuals to appointments and community activities.
• Document daily activities and medication administration in the EHR.
• Follow applicable regulations and safety procedures.
• Communicate effectively with team and participate in meetings and trainings.
