Job summary

Oak Hill is seeking a Direct Support Professional to provide person-centered support in a residential setting in Enfield, CT. The role focuses on daily living activities, health monitoring, medication administration, and community inclusion, with a schedule of Saturdays and Sundays from 3pm to 11pm and a base rate of $20.39 per hour.

You will help individuals with independence, nutrition, mobility, and safety while documenting care and coordinating with a team.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment.
• Commitment to continuity of care, including occasional shifts beyond schedule.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene needs per individual plans.
• Provide continence care as needed.
• Encourage independence and skill development where possible.
• Monitor health and document vital signs when required.
• Administer medications and maintain medication records per training and directives.
• Follow health care plans and support overall well-being.
• Transport individuals to appointments and activities using agency vehicles.
• Encourage participation in community life and personal interests.
• Prepare meals and snacks according to menus and dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and health information in the EHR.
• Follow emergency procedures and safety protocols.
• Communicate effectively with team, individuals served, families, and stakeholders.
