Job summary

Oak Hill is seeking a Direct Support Professional (Substitute/Per-Diem) in West Hartford, CT. You will support adults with intellectual and developmental disabilities in a residential setting, performing daily living activities, health monitoring, and medications administration as trained.

The role requires 18+ years old, a HS diploma or GED, and a valid driver’s license. Per Diem shifts start at 15 hours/month with wage $18.50–$19.40/hour; male applicants only are considered at this time.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Able to pass background checks, fingerprinting, and pre-employment health screenings.
• Complete required training programs and maintain certifications.

Responsibilities

• Support individuals in daily living activities, including grooming, hygiene, and mobility.
• Monitor health information and document health-related data.
• Administer medications per training and policies.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and assist with mealtime support.
• Maintain accurate documentation in the EHR and ensure regulatory compliance.
• ProvideEmergency procedures and safety protocols; apply First Aid/CPR when required.
• Collaborate with team, attend trainings, and participate in ongoing professional development.
