Job summary

Oak Hill in Burlington, CT is seeking a Direct Support Professional Substitute to join our residential group home team. This per-diem DSP role provides person-centered support to adults with IDD, promoting independence, dignity, and community connection.

You'll perform daily living assistance, health monitoring, medication administration, nutrition support, mobility help, and thorough documentation in our electronic health record.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license with ability to transport.
• Ability to complete background checks and health screenings.
• Ability to complete required training programs and certifications.
• Commitment to a safe, professional work environment.
• Willingness to stay beyond shift when coverage is needed.
• Experience as DSP/CNA or related role preferred but not required.

Responsibilities

• Provide daily living assistance and personal care.
• Monitor health and document vital signs and health information.
• Administer medications per training and health plans.
• Support nutrition and mealtime needs.
• Assist with mobility, transfers, and use of adaptive equipment.
• Document activities, health data, incidents, and medications in EHR.
• Follow emergency procedures and safety protocols.
• Collaborate with team and participate in trainings.
