Responsibilities

  • Initiate and conduct intake and engagement processes for assigned Persons Served within 72 hours.
  • Assess treatment and support needs on an ongoing basis.
  • Determine rehabilitative service needs and develop Medicaid Rehab Option billable treatment plans.
  • Complete Mental Status Exams, Clinical Formulation-Interpretive Summaries, treatment plans, reviews and revisions.
  • Lead intervention teams comprising outreach, nursing, housing, addiction recovery, peer, employment and family support staff.
  • Oversee service delivery, recovery goals, peer and family support, transitions and discharge readiness.
  • Develop and deliver individualized services and supports for young adults and seniors.
  • Provide staff training using trauma-informed, recovery-oriented, person-centered and strengths-based approaches.
  • Interface with Emergency Services, employment coordinators and care coordination entities.
  • Conduct face-to-face visits within 48 hours after specified emergency, justice, homelessness or hospitalization events.

Job description

  • Conduct Treatment Plan meetings and plan for discharge
  • Lead intervention teams comprising outreach, nursing, housing, addiction recovery, peer, employment and family support staff
  • Oversee service delivery, recovery goals, peer and family support, transitions and discharge readiness
  • Develop and deliver individualized services and supports for young adults and seniors
  • Collaborate with team members to provide recovery, community, addiction, health and family services
  • Promote healthcare, employment access, safe housing, community tenure and self-sufficiency
  • Assign and direct Outreach Workers and train/direct GLE/SIE staff
  • Train, supervise, mentor and model evidence-based clinical and recovery-oriented interventions
  • Ensure integration of team and residential staff interventions and thorough documentation
  • Provide staff training using trauma-informed, recovery-oriented, person-centered and strengths-based approaches
  • Provide outreach to disengaged Persons Served
  • Support care, housing and life-stage transitions
  • Develop and update Safety Plans and Crisis Prevention and Response Plans
  • Interface with Emergency Services, employment coordinators and care coordination entities
  • Conduct face-to-face visits within 48 hours after specified emergency, justice, homelessness or hospitalization events
  • Visit assigned GLE/SIE locations twice weekly when assigned
  • Provide field supervision to Outreach Workers
  • Receive role-based clinical supervision and perform other assigned duties
Requirements
  • Master's degree required in Social Work, Psychology, Counseling Psychology or other LCSW or LMHC or other LPHA licensure eligible program
  • LCSW, LICSW, LMHC credentials valid in Massachusetts required
  • Clinical experience working with people with psychiatric disability and/or co-occurring disorders preferred
  • Preference given to bilingual/bicultural applicants and applicants with lived experience of psychiatric conditions
  • Must have access to an operational and insured vehicle and be willing to use it to transport Person Served
  • Must be at least 21 years of age
  • Must pass CORI, driving record, reference, educational and professional licensure checks and a multi-state background check
  • Must become CPR and First Aid certified within two weeks of employment and MAP certified within 150 days of employment
  • Knowledge of clinical treatment principles, modalities, best practices and evidence-based practices
  • Knowledge of engagement, crisis prevention, crisis intervention and risk management strategies
  • Knowledge of recovery-oriented, person-centered, strengths-based, trauma-informed and culturally competent approaches
  • Knowledge of emergency services, acute care systems, community-based mental health and care coordination resources
  • Ability to formulate effective Treatment Plans and train paraprofessional staff
  • Ability to collaborate effectively with team members
  • Knowledge of formal and informal assessment practices
  • Knowledge of documentation standards and requirements
  • Valid US driver's license held for at least one year, adequate insurance if using a personal vehicle, and safe driving record
