  • Complete logs, timesheets, and forms; conduct crisis intervention as needed.

Job description

  • Assist in developing, implementing, and documenting individual treatment support plans
  • Perform daily tasks with persons served, including rehabilitative and behavioral interventions, appointment scheduling, progress monitoring, transportation, and related support
  • Provide role modeling and support, helping persons served integrate into their communities
  • Assess physical and emotional status and report changes to a supervisor
  • Ensure program site safety and maintenance
  • Participate in daily living and housekeeping activities, including cooking, meal planning, grocery shopping, lawn mowing, snow shoveling, and garbage removal
  • Comply with corporate and divisional policies, safe work practices, workers’ compensation, universal precautions, and exposure control requirements
  • Store, administer, document, and dispose of medication according to MAP requirements, funding-source regulations, and company policy
  • Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
  • Perform crisis intervention as needed
  • Provide current contact information to the Relief Pool Coordinator or Human Resources staff
  • Perform other related duties as required
Requirements
  • One to three months of experience in a human services setting
  • Experience may be substituted for academic training in some cases
  • High school diploma or equivalent required
  • Valid US driver’s license must be presented at employment if driving
  • At least 21 years of age if driving
  • Valid US driver’s license maintained for at least six months if driving
  • Ability to pass a driver screening background check if driving
  • Safe driving record if driving
  • Adequate insurance if using a personal vehicle
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with daily living tasks
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required vision abilities including close, distance, color, peripheral, and depth perception, plus ability to adjust focus
  • First Aid required within two weeks of hire
  • CPR required within two weeks of hire
  • MAP required within 150 days of hire
  • Safety Care required within 90 days of hire
  • Knowledge of advocacy, communication, empowerment, self-advocacy, physical personal management, household management, and crisis intervention techniques
  • Sensitivity to cultural, religious, racial, disability, and gender issues
