Job summary

Jobtailor is seeking a dedicated human services assistant to support clients in daily living tasks and community integration. You will help with treatment planning, monitor progress, and assist with transportation and daily activities.

The role requires strong communication, safety awareness, and the ability to work with a team to maintain program standards. A valid driver's license and a high school diploma are required, with training on MAP, Safety Care, and First Aid/CPR within specified

Qualifications

  • One to three months of experience in a human services setting.
  • Experience may substitute for academic training in some cases.
  • High school diploma or equivalent required.
  • Valid US driver’s license presented at employment if driving.
  • At least 21 years of age if driving.
  • Valid US driver’s license maintained for at least six months if driving.
  • Pass a driver screening background check if driving.
  • Maintain a safe driving record if driving.
  • Adequate insurance if using a personal vehicle.

Responsibilities

  • Assist in developing, implementing, and documenting individual treatment support plans.
  • Perform daily rehabilitative and behavioral interventions, monitoring, appointment scheduling, progress monitoring, transportation, and related tasks.
  • Provide role-model support and help persons served become integrated into their communities.
  • Assess physical and emotional status and report changes to a supervisor.
  • Ensure program safety and maintenance.
  • Participate with persons served in cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal.
  • Comply with corporate and divisional policies, safe work practices, workers’ compensation, universal precautions, and exposure control.
  • Store, administer, document, and dispose of medication according to MAP requirements, regulations, and company policy.
  • Complete communication logs, community membership logs, chore schedules, timesheets, and application forms.
  • Perform crisis intervention as needed.
  • Maintain current contact information with the Relief Pool Coordinator or Human Resources.
  • Perform other related duties as required

  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with daily living tasks
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required vision abilities including close, distance, color, peripheral, and depth perception, and ability to adjust focus
  • First Aid required within two weeks of hire
  • CPR required within two weeks of hire
  • MAP required within 150 days of hire
  • Safety Care required within 90 days of hire

Demonstrates the ability to assist clients with daily living tasks while ensuring safety and compliance with regulations. Proficient in developing treatment support plans and performing rehabilitative interventions.
