SCAN Health Plan seeks a part-time Peer Advocate to support at-risk members by sharing experiences and coaching behavior changes to improve health outcomes. The role involves outbound outreach, building trust, and guiding members through care options.

Candidates should be SCAN members, possess strong communication and organizational skills, and be comfortable working in a fast, evolving environment. This is a temporary, virtual-training-friendly position with paid hourly wages.

Formación

• High School Diploma or equivalent preferred.
• Current SCAN Membership required.
• Proficient with operating a computer, including logging into and maneuvering around multiple systems such as online timesheets (time and attendance) and call center software.
• Excellent communication skills and strong interpersonal and written/verbal abilities.
• Ability to work independently as well as part of a team.
• Strong organizational skills.

Responsabilidades

• Make outbound calls to SCAN members based on identified health concerns and quality initiatives.
• Build rapport with members by sharing personal SCAN member experiences to enhance health education.
• Coach members to implement strategies that minimize risks associated with common conditions.
• Encourage and provide assistance to members who may face barriers to seeking care.
• Document all required information in the care management system during calls.
• Maintain client confidentiality at all times.
