Job summary

SCAN Health Plan is seeking a remote part-time Peer Advocate to support members facing health challenges. You will use your own SCAN membership experience to connect, guide, and empower individuals toward recovery and wellness in a healthcare/social service context.

This role requires SCAN Membership, strong communication, and ability to work remotely with autonomous and team collaboration. HS diploma and computer skills are essential.

Qualifications

• High School Diploma or equivalent.
• Current SCAN Membership is required.
• Proficient with a computer and multiple systems (timesheets and call center software).
• Strong MS Office and internet navigation skills.
• Excellent communication; able to articulate personal SCAN membership experience.
• Bi-lingual in Spanish, Korean or Chinese is a plus.
• Strong interpersonal, written and verbal communication.
• Ability to work independently and in a team; strong organizational skills.
• Valid driver's license, automobile insurance and reliable transportation.

Responsibilities

• Make outbound calls to members based on health interventions or quality initiatives.
• Build rapport using personal SCAN membership experience to enhance health education.
• Coach members to create strategies that minimize risks for health conditions.
• Encourage and assist members who may face obstacles to seeking care.
