Job summary

Affirmed Home Care is seeking compassionate LPNs to deliver in-home pediatric nursing, one-on-one care, and family-centered support across Fairfield County, CT. Join a team focused on safe, high-quality home health services for children, with flexible per diem shifts and comprehensive onboarding.

Qualifications

• Active Connecticut LPN license.
• Current BLS Certification.
• Minimum of one year of pediatric nursing experience preferred (home care, hospital, or acute care).
• Authorized to work in the United States.
• Valid driver’s license preferred.
• Current physical completed within the last 12 months.
• Current TB screening (PPD or QuantiFERON within one year) or chest X-ray, if applicable.

Responsibilities

• Provide one-on-one pediatric nursing care in patients’ homes.
• Coordinate and monitor in-home treatments and medications.
• Collaborate with families and clinical team to adjust care plans as needed.
