Job summary

Affirmed Home Care is seeking a licensed practical nurse (LPN) to provide one-on-one pediatric nursing care in the homes of clients in Fairfield County, Connecticut. The role offers flexible per diem shifts, including day, evening, overnight and weekend slots, with competitive pay and a sign-on bonus.

Qualifications

• Active Connecticut LPN license.
• Current BLS Certification.
• Minimum one year pediatric nursing experience preferred (home care, hospital, or acute care).
• Authorized to work in the United States.
• Valid driver's license preferred.
• Current physical examination completed within the past 12 months.
• Current TB screening (PPD or QuantiFERON completed within the last year) or chest X-ray documentation, if applicable.

Responsibilities

• Provide one-on-one pediatric nursing care in patients' homes.
• Support care plans and ensure patient safety during visits.
• Coordinate with families and healthcare providers to maintain continuity of care.
