Job summary

Affimmed Home Care is seeking compassionate Licensed Practical Nurses (LPNs) to provide one-on-one pediatric home care across Fairfield County, CT. Per diem shifts include day, evening, overnight, weekend, with flexible scheduling and ongoing clinical support.

Requirements include active CT LPN license, current BLS, and at least one year of pediatric nursing experience. Per diem work offers flexibility, sign-on bonus, and a supportive team to help you succeed.

Qualifications

• Active Connecticut LPN license.
• Current BLS Certification.
• Minimum of one year of pediatric nursing experience preferred (home care, hospital, or acute care).
• Authorized to work in the United States.
• Valid driver's license preferred.
• Current physical completed within the last 12 months.
• Current TB screening (PPD or QuantiFERON within one year) or chest X-ray, if applicable.

Responsibilities

• Provide one-on-one skilled nursing care for pediatric clients in patients' homes across Fairfield County, CT.
• Support pediatric patients and families with care plans and ongoing assessment.
• Collaborate with nursing and administrative teams to ensure high-quality home care services.
