Job summary

Angels of Care Pediatric Home Health is seeking an Intake Coordinator for an on-site role at our Sherman, TX office. You will manage new patient referrals, gather information, verify benefits, and coordinate intake scheduling to ensure a smooth start of care for each client.

This position serves as a key contact for families, referral sources, providers, insurers, and internal teams throughout the intake process, promoting organized workflows and timely starts of care.

Qualifications

• High School diploma or equivalent required
• Administrative/healthcare experience preferred
• Excellent communication and organizational skills
• Bilingual in Spanish preferred

Responsibilities

• Handle new patient referrals from hospitals, doctors, case managers, and other sources
• Collect and verify information for new pediatric home health patients
• Screen inquiry calls and schedule intakes with families or referrals
• Verify Medicaid and private insurance benefits
• Coordinate with case managers and clinical teams to complete intakes
• Maintain accurate documentation and follow intake protocols
• Support onboarding by ensuring all required paperwork is completed
• Communicate with families and staff to coordinate a timely start of care
