Job summary

Angels of Care Pediatric Home Health is seeking an Intake Coordinator to manage new patient referrals and coordinate onboarding for pediatric home health clients. This on-site role requires organization, communication, and collaboration with families, referral sources, and clinical teams to ensure a smooth start of care.

You will verify insurance benefits (Medicaid and private) and support scheduling, documentation, and interdepartmental coordination.

Qualifications

• Experience in administrative, healthcare, patient access, referral coordination, scheduling, or insurance verification is preferred.
• Excellent communication, follow-up, and organizational skills are required.
• Bilingual in Spanish preferred

Responsibilities

• Handle new patient referrals from hospitals, doctor’s offices, case managers, and other referral sources.
• Collect and verify information for new pediatric home health patients.
• Screen inquiry calls and follow up with families or referral sources to schedule intakes.
• Verify insurance benefits, including Medicaid and private insurance.
• Coordinate with case managers, clinical teams, and internal departments to assign and complete intakes.
• Maintain accurate documentation and follow established intake protocols.
• Support new client onboarding by ensuring all required paperwork and information are completed.
• Communicate with families, internal staff, and community healthcare partners to help coordinate a timely start of care.
