Job summary

Molina Healthcare, Inc. is seeking a dedicated Care Manager RN in Lakeland, FL to support care coordination across the continuum. You will assess members, develop and monitor care plans, and collaborate with physicians, caregivers, and the care team to achieve outcomes.

You will conduct telephonic, in-person, or home visits, manage an active caseload, and navigate EMR systems while addressing barriers to care. Travel between 25% and 40% may be required based on contracts.

Qualifications

• 2+ years in health care, preferably in care management.
• RN with active license in state of practice.
• Valid driver license, reliable transportation for travel.
• Understanding of EMR and HIPAA requirements.
• Knowledge of community resources and care planning.

Responsibilities

• Completes comprehensive member assessments per timelines to identify care management eligibility.
• Develops and implements care coordination plans with the care team and members.
• Performs telephonic, in-person, or home visits as required.
• Monitors and documents progress, adjusting care plans as needed.
• Manages ongoing member caseload with proactive outreach.
• Promotes integration of health and social services for continuity of care.
