Qualifications

Responsibilities

• Supports care management activities and coordinates with multidisciplinary team.
• Develops and implements care plans with member and providers.
• Handles telephonic, in-person, or home visit follow-ups as needed.
• Monitors progress, documents interventions, and updates goals.
• Maintains ongoing member caseload and outreach.
• Facilitates care integration across behavioral health, LTSS, and community resources.
• Supports ICT meetings and collaborates with care managers.
• Uses motivational interviewing to educate and motivate members.
• Identifies barriers to care and provides solutions.
• Travel 25–40% based on state requirements.
