Job summary

Molina Healthcare, Inc. is seeking a Care Coordinator to support care management activities and collaborate with a multidisciplinary team to deliver integrated member care across settings.

You will assess, plan, implement, and monitor care plans, coordinating with members, caregivers, physicians, and other health professionals. Travel may be required to meet state contracts and ensure quality outcomes.

Qualifications

• At least 2 years experience in health care, preferably in care management or related setting.
• Clinical licensure and/or certification required ONLY if required by state contract or licensing mandates.
• Valid and unrestricted driver’s license and reliable transportation for travel requirements.
• Knowledge of community resources and ability to navigate services.

Responsibilities

• Complete member assessments within regulated timelines and identify care coordination eligibility.
• Develop and implement care plans in collaboration with member, caregivers, and health professionals.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor care plan effectiveness and document interventions and outcomes.
• Maintain ongoing member caseloads and proactive outreach.
• Coordinate integration of behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to support and motivate member change.
• Identify barriers to care and provide coordination assistance.
• Collaborate with licensed care managers as required.
• Travel 25-40% may be required depending on state requirements.
