Molina Healthcare, Inc. seeks a Care Manager to support care management and coordination across the continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes while contributing to cost-effective, high-quality care.

The role involves 25-40% local travel and requires experience in healthcare, strong communication, and the ability to work independently in diverse settings. A valid driver’s license and tool proficiency in Microsoft Office are expected.

Formación

• At least 2 years experience in health care, preferably in care management, or equivalent combination of relevant education and experience.
• Clinical licensure and/or certification required only if required by state contract or licensing mandates.
• Valid driver's license, reliable transportation and adequate auto insurance for job-related travel requirements.

Responsabilidades

• Completes assessments of members per timelines and determines eligibility for care coordination.
• Develops and implements care plans with member and care team to address needs and goals.
• Conducts telephonic, face-to-face, or home visits as required.
• Monitors progress, documents interventions and adjusts plan as needed.
• Maintains ongoing member caseload and outreach for management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
