Qualifications

• At least 2 years in health care, preferably in care management
• Valid driver's license and reliable transportation
• Knowledge of community resources
• Ability to operate proactively and be detail oriented
• Strong verbal and written communication skills
• Bachelor's degree may be required in some states (health care field)

Responsabilités

• Completes assessments of members per regulated timelines and determines care coordination eligibility
• Develops and implements care plans with member and care team
• Conducts telephonic, face-to-face or home visits as required
• Monitors care plans, documents interventions, and adjusts as needed
• Maintains ongoing member caseload for outreach and management
• Promotes integration of services (behavioral health, LTSS, home resources)
• Facilitates interdisciplinary care team (ICT) meetings
• Uses motivational interviewing to educate and motivate change
• Assesses barriers to care and provides assistance
• Collaborates with licensed care managers as needed
• Travel 25-40% may be required
