Qualifications

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with the care team and member.
• Conduct telephonic, in-home visits, or clinic visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain an ongoing member caseload with proactive outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and multidisciplinary collaboration.
• Educate and motivate members using motivational interviewing techniques.
• Identify barriers to care and coordinate solutions with the member.
