Responsibilities include assessments, care plans, and home visits as needed.

You will monitor progress, promote service integration (behavioral health, LTSS, and community resources), and participate in ICT meetings. Travel 25–40% may be required.

Qualifications

• Active RN license in the state of practice.
• Minimum 2 years of healthcare experience, preferably in care management.
• Knowledge of EMR and HIPAA requirements.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with the member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and document care plan progress and adjust as needed.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and interdisciplinary collaboration.
• Provide coaching and support to peers as needed.
• Care managers may handle complex cases and medication reconciliation.
