Job summary

You will conduct assessments, develop care plans, perform visits, monitor progress, and support care integration with LTSS and community resources; travel 25-40% as required.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Valid driver’s license and reliable transportation.
• Excellent verbal and written communication; proficient with MS Office.
• Experience with severe mental health conditions, substance use and foster care.

Responsibilities

• Complete comprehensive behavioral health assessments per regulated timelines.
• Develop and implement care plans with member, caregiver and healthcare professionals.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plan effectiveness, document progress and adjust as needed.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate and motivate members using motivational interviewing and clinical guidelines.
• Identify barriers to care and provide coordination assistance.
• May provide consultation and resources to peers as needed.
• Travel 25-40% locally based on state/contractual requirements.
