Molina Healthcare, Inc. seeks a care management professional to provide coordination across the continuum of care for members. You will work with an interdisciplinary team to ensure quality, cost-effective outcomes and engage in member assessments, care planning, and ongoing monitoring.

The role requires travel (25-40%), strong communication, and knowledge of community resources. A health-care related degree and relevant licensure may be required depending on state contracts.

Formación

• 2+ years in health care, preferably in care management or related setting.
• Valid driver's license, reliable transportation, and auto insurance for travel.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong communication and time-management skills.
• Proficiency with Microsoft Office.

Responsabilidades

• Completes assessments of members per regulated timelines and identifies candidates for care coordination/care management.
• Develops and implements care plans in collaboration with members and care teams.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plans, documents interventions, and suggests changes as needed.
• Maintains ongoing member caseload and outreach for management.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
