Job summary

This hybrid role blends telephonic, virtual, and field-based engagement. Responsibilities include assessments, care plans, coordinating services, travel locally 25-40%, and maintaining compliant documentation.

Qualifications

• Minimum 2 years of healthcare, care management, case management, behavioral health, or related experience.
• Active, unrestricted Florida license in LVN/LPN, RN, or Licensed Behavioral Health Clinician (LCSW, LMSW, LPC, LMFT, LPCC, PsyD, PhD, or equivalent).
• Strong communication, critical thinking, and care coordination skills.
• Proficiency with EMR systems and Microsoft Office.
• Valid driver's license, reliable transportation, and ability to travel locally (25% to 40%).

Responsibilities

• Conduct clinical and/or behavioral health assessments.
• Develop and manage individualized care plans.
• Coordinate medical, behavioral health, LTSS, and community services.
• Monitor member progress and adjust interventions as needed.
• Facilitate interdisciplinary care team collaboration.
• Conduct telephonic, face-to-face, and home visits as required.
• Address barriers to care and support members in achieving health goals.
• Maintain documentation and regulatory compliance.
