Molina Healthcare, Inc. is seeking a Care Manager to support care management and care coordination activities.

The role involves collaborating with a multidisciplinary team to drive integrated member care across the continuum and ensure progress toward outcomes while contributing to cost-effective strategies. The ideal candidate has healthcare experience, a valid driver’s license, strong communication, and the ability to work independently.

Formación

• 2+ years experience in health care, care management preferred.
• License/certification required only if state contract mandates it.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and care coordination.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication skills.

Responsabilidades

• Complete member assessments within regulated timelines and determine eligibility for care coordination.
• Develop and implement care plans with member and care team to address needs and goals.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document interventions and outcomes.
• Maintain ongoing member caseload and perform outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaborate with care teams.
• Address barriers to care and assist members with resources.
• Collaborate with licensed care managers as needed.
• Travel 25-40% as required by state contracts.
