The role emphasizes ensuring progress toward outcomes and contributing to quality, cost-effective care. The position includes assessing members, developing care plans with the care team, and monitoring progress, with 25-40% local travel.

Qualifications

• At least 2 years experience in health care, care management or related field.
• Valid and unrestricted driver's license and reliable transportation.
• Ability to work independently and manage time across multiple tasks.
• Knowledge of community resources and ability to navigate diverse settings.
• Proactive communication and ability to collaborate with multidisciplinary teams.

Responsabilités

• Completes member assessments within regulated timelines and identifies care coordination eligibility.
• Develops and implements care plans with member, caregivers, and clinicians.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and progress.
• Maintains ongoing member caseload with regular outreach and management.
• Coordinates integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides assistance to address concerns.
