Qualifications

• Active RN license in state of practice.
• 2+ years healthcare experience, care management preferred.
• Familiarity with HIPAA and EMR
• Willingness to travel 25–40% as required.

Responsibilities

• Performs comprehensive member assessments per timelines and determines care management eligibility.
• Develops and implements care coordination plans with members, families, and providers.
• Conducts telephonic, in-person, or home visits as required.
• Monitors progress and adjusts care plan to meet outcomes.
• Maintains caseload and coordinates services across LTSS, behavioral health and community resources.
• Facilitates ICT meetings and collaborates with team; educates members using motivational interviewing.
