Qualifications

• At least 2 years experience in health care, care management or related setting.
• Valid driver's license and reliable transportation.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Excellent time-management and prioritization.
• Strong verbal and written communication.
• Proficiency with Microsoft Office applications.
• In some states, a health care related Bachelor's degree may be required.

Responsibilities

• Complete member assessments per regulated timelines to determine eligibility for care coordination.
• Develop and implement care plans with member and care team.
• Conduct telephonic, face-to-face or home visits.
• Monitor care plans and document interventions and goals achieved.
• Maintain ongoing member caseload with regular outreach and management.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and collaboration.
• Use motivational interviewing and Molina clinical guidelines.
• Assess barriers to care and provide care coordination assistance.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required.
