Job summary

Molina Healthcare, Inc. is seeking a Care Manager to support care coordination across the continuum. You will work with members, caregivers and health professionals to develop and implement care plans, oversee follow‑ups, and promote integrated services that improve outcomes while controlling costs.

Required are 2+ years in health care or care management, a valid driver’s license, and strong communication skills. Travel up to 40% may be necessary depending on state requirements.

Qualifications

• 2+ years experience in health care or care management
• Clinical licensure and/or certification as required by state contract/regulation
• Valid driver’s license and reliable auto insurance for travel
• Solid knowledge of community resources and ability to work independently

Responsibilities

• Completes assessments to determine care coordination eligibility.
• Develops and implements care plans with member, caregiver, physician and other care professionals.
• Conducts telephonic, in‑person, or home visits as required.
• Monitors care plans, documents interventions and evaluates outcomes.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to support change.
• Assesses barriers to care and provides assistance to members.
• Travel may be 25–40% depending on state/contractual requirements.
