This role collaborates with physicians and families, drives performance improvement, and supports recruitment while fostering a collaborative, compliant work environment across departments.

Qualifications

• Care management experience demonstrated in healthcare settings.
• RN or Social Work licensure in state of practice required.
• ACM or CCM certification must be obtained within 2 years of hire.

Responsibilities

• Provide leadership and oversight of patient care operations to ensure safe, high-quality, evidence-based care that meets regulatory and professional standards.
• Manage department staffing, scheduling, and resource allocation to maintain appropriate patient care coverage.
• Supervise, coach, and evaluate nursing staff performance and clinical competency.
• Coordinate onboarding, training, and ongoing education initiatives for staff.
• Lead performance improvement, quality assurance, and documentation reviews to enhance patient outcomes.
• Build relationships with physicians, patients, families, and interdisciplinary teams to support care.
• Participate in recruitment, interviewing, and retention efforts for department members.

Skills

Care Management experience

RN licensure or Social Work licensure

ACM/CCM certification within 2 years
