Job summary

Shriners Children’s Hospital in Spokane, WA is seeking a PRN RN Care Manager to join our team. You will assess, plan, implement and follow up with patients and families across the continuum of care, using face-to-face and telephonic encounters to build and monitor treatment plans.

The role requires a WA RN license, a Bachelor’s in Nursing and at least 3 years of nursing experience; Masters in Nursing and Case Management Certification are preferred. Bilingual Spanish is a plus.

Qualifications

• RN license in Washington state required.
• Bachelor's degree in Nursing required as base qualification.
• Minimum 3 years of nursing experience.
• Master's in Nursing and Case Management Certification are preferred.
• 1 year of pediatric care management experience is preferred.
• Bilingual Spanish is a plus.

Responsibilities

• Provide assessments, planning, interventions and follow-up for patients and families.
• Coordinate care plans with interdisciplinary team and primary care/medical home.
• Advocate for patient safety, health, and rights.
• Collaborate with community services to arrange necessary resources.
