Job summary

Qualifications

• 2+ years of care management experience required, in a pediatric setting preferred.
• RN or social work licensure in practice state required.
• ACM or CCM certification due within 2 years of hire.
• Master's degree in a related field is preferred but not required.

Responsibilities

• Oversees nursing, social work, interpretive services, and resource coordination in care management.
• Plans and coordinates staffing schedules to ensure adequate patient care coverage.
• Leads performance improvement, policy updates, and department environment enhancements.
• Participates in staff interviews and ensures clinical competency through evaluations.
• Supports Joint Commission readiness and collaborates with the multidisciplinary team.
