Qualifications

• BSN or bachelor’s degree in Social Work.
• Minimum 2 years of care management experience.
• RN or Social Work license in state of practice.
• ACM or CCM certification required within 2 years of hire.
• Masters degree in related field preferred.

Responsibilities

• Oversee patient care to ensure safety, efficiency, and evidence-based practice.
• Coordinate staff schedules to ensure adequate patient coverage.
• Promote staff engagement and participate in committees or projects.
• Recommend changes to policies, procedures, and department environment to improve service.
• Evaluate clinical competency and participate in performance improvement.
• Participate in interviewing new team members.
• Coordinate onboarding training and competencies.
• Serve as a clinical resource to staff.
• Assist staff in resolving patient care challenges.
• Coordinate emergency preparedness of the department.
• Support Joint Commission readiness.

Education

BSN or Bachelors in Social Work

2 years Care Management experience

RN or Social Work License in state of practice

ACM or CCM Certification required within 2 years of hire
