Job summary

The Coordinating Center in Maryland is seeking a Clinical Care Coordinator for the REM program. This full-time, hybrid role blends telework with client visits across lower Prince George's County, focusing on Medicaid recipients under 21 with complex medical needs.

You must have an active Maryland LMSW/LCSW-C or RN license, at least two years in case management, pediatric experience preferred, and the ability to obtain CCM certification within two years.

Qualifications

• Active Maryland Social Work license (LMSW/LCSW-C) or Maryland RN license required.
• At least two years of case management experience.
• Pediatric experience preferred.
• Ability to obtain CCM certification within two years of employment.

Responsibilities

• Develop and implement individualized care management plans with client and caregiver input.
• Coordinate and navigate medical and home/community services for complex cases.
• Research cost-effective care alternatives when appropriate.
• Guide care delivery using assessments, planning, education, advocacy, and evaluation.
• Communicate with providers, insurers, and the health care team.
• Document care management activities in electronic databases while protecting PHI.
