Job summary

Larimer County Department of Health and Environment seeks a Care Coordinator to support children and youth with special health care needs and their families. You’ll identify resources, help overcome barriers, and connect families to services that best meet their needs.

You will develop individualized action plans, coordinate services, make referrals, and assist families navigating Medicaid, disability, health care, and other supports.

Qualifications

• Bachelor's degree or equivalent with education/experience substitution allowed.
• Valid driver's license required by date of hire.
• Complete training and/or certification as mandated by the assigned program.

Responsibilities

• Provide individualized care coordination and family support by connecting families to appropriate community resources and services.
• Schedule and complete targeted outreach with local providers and community partners; participate in coalition work.
• Assist families with navigating Medicaid and systems relating to disabilities.
• Maintain timely, accurate, and thorough documentation.
• Collaborate with families, community providers, and partner agencies; attend community meetings.
• Protect confidentiality and follow policies, regulations, and program requirements.
