Job summary

Qualifications

Responsibilities

• Develop and maintain individualized care plans with desired outcomes
• Document client interactions promptly in the database within 48 hours
• Collaborate with interdisciplinary team and family members
• Deliver care through on-site visits and virtual engagements
• Maintain HIPAA compliance and accurate records
