Qualifications

Responsibilities

• Manage a caseload of medically fragile enrollees for care coordination.
• Assess enrollees' needs and develop comprehensive care plans.
• Conduct home visits as necessary based on enrollee acuity.

Education

Associate’s Degree in Nursing (Required)
