Job summary

St. Mary’s Healthcare System for Children is seeking a Health Homes Care Coordinator to oversee the coordination of pediatric care across services, ensuring comprehensive family-centered plans.

This role interfaces with physicians, nurses, social workers, and external agencies to secure timely care. The position requires a Bachelor’s degree in a health/human services field and a minimum of two years in case management or medical care coordination for children with chronic conditions.

Qualifications

• Bachelor’s degree in health or related field required
• Minimum 2 years of case management or care coordination experience with chronic conditions
• Experience in Early Intervention ongoing service coordination preferred
• Bilingual Spanish speaking preferred
• Valid NYS ID and/or driver’s license; willingness to travel across designated counties

Responsibilities

• Completes CANS assessment and develops a comprehensive plan of care
• Builds relationships with families and care teams to coordinate services
• Ensures visits/contacts to provide coordinated services
• Works with Medicaid provider and network enrollment
• Participates in team meetings to review and implement care plans
• Provides updates to management and suggests process improvements
• Attends team patient care conferences for complex cases
• Meets monthly care management data entry and documentation requirements
• Ensures productivity statistics comply with care management requirements
• Participates in required trainings and ongoing assessments
