Job summary

Plan A Health is seeking a Personal Care Aide to provide client care in a home care setting. You will follow the aide plan, complete tasks via EVV, and assist with daily personal care using walkers and wheelchairs.

You will help with safety, infection control, and confidentiality while performing light household tasks as needed. A high school diploma and at least 6 months of caregiving are preferred, and you must be 18+.

Qualifications

• High School diploma preferred; ability to read and write required.
• At least 6 months of validated caregiving experience.
• Must be 18 years or older.
• Ability to encourage self-help and support independence.
• Strong adherence to safety, infection control, and confidentiality protocols.

• Monitor client conditions, report changes, and notify supervisors of concerns or unsafe environments.
• Assist with self-administration of medications (without directly administering).
• Adhere to scope of practice, seek guidance when necessary, and follow policies and procedures.
