Job summary

Responsibilities

• Facilitate patient placement as directed by the Patient Logistics Manager, PLRN, and physicians.
• Coordinate transfers to appropriate facility and unit.
• Identify problems and escalate to prevent barriers to timely placement and transfer.
• Communicate with referring and receiving facility staff to ensure timely placement.
• Document activities clearly to promote team communication.
• Use effective communication with care team members and providers.
