Job summary

Mountain Region Clinical Resources is seeking a Patient Placement Coordinator to manage patient placement in acute care settings, ensuring regulatory compliance and accurate data entry in EPIC. This role emphasizes meticulous organization and clear communication to support timely patient flow.

You will coordinate bed assignments using placement criteria, document in EPIC in real time, and foster strong relationships with hospital staff and stakeholders to optimize care transitions.

Qualifications

• High School Diploma or G.E.D. required.
• 1–2 years direct clinical or admin healthcare experience.
• Working knowledge of medical terminology.

Responsibilities

• Follow diagnosis-based placement criteria and involve RN Census as needed.

Job Summary and Responsibilities

As our Patient Placement Coordinator, you will coordinate all processes, support, communication, and documentation for placing patients in the appropriate facility care setting, meticulously upholding all regulatory requirements. Every day you will follow patient placement criteria, accurately enter all required data into the EPIC documentation system, and continuously update key stakeholders on timelines, ensuring efficient and compliant patient flow. To be successful in this role, you will possess exceptional organizational skills, meticulous attention to detail in data entry and documentation, a strong understanding of patient placement criteria and regulatory requirements, and excellent communication abilities to manage patient flow effectively.

• Coordinates patient bed assignment utilizing criteria and engaging RN Census Supervisor if clinical judgment is needed for placement decisions.
• Accurately follow diagnosis based clinical criteria for bed placement.
• Utilize excellent communication skills in all communications between involved hospital staff, while relaying information accurately.
• Accurately document and enter all required information into EPIC as it occurs, for creation of realtime data collection.
• Has a strong understanding of the acute care facilities and their units and care settings.
• Ability to develop and maintain relationships with stakeholders for patient placement within facilities.

Job Requirements

Required

• High School Diploma/G.E.D.
• One (1) to two (2) years of direct clinical experience, or one (1) to two (2) years of full time direct administrative healthcare experience (such as Nursing Unit Secretary or Patient Access Representative)
• Working knowledge of medical terminology
