Job summary

Mercy is seeking a Night Shift Transfer Center Coordinator to support patient admissions, discharges, and inter-facility transfers. You will monitor census, maintain bed data, and coordinate with clinical and referring staff to ensure timely patient movement.

Responsibilities include leading team flow, documenting calls, and assisting with prioritization during peak periods. Requires strong communication and healthcare terminology knowledge.

Qualifications

• High school diploma or GED required.
• Associate's degree or equivalent experience preferred.
• Knowledge of Medical Terminology required.
• Strong written and oral communication skills.
• Ability to prioritize workload under time constraints.
• Healthcare experience preferred but not required.

Responsibilities

• Directs the daily flow of communication and collaboration between all active team members to ensure the highest level of service is being achieved.
• Leads process improvement initiatives, including workflow development and business strategies.
• Organizes key metrics into standard leader reporting.
• Understands Mercy One Patient Logistics strategy and service lines.
• Develops strategies to maintain and support team satisfaction.
• Coordinates and communicates accurate census and bed availability information.
• Assists with transfer center calls and communication priorities for the team.
• Supports and maintains flow of incoming calls between the transfer center team.
• Logs calls and communicates with customers in the electronic documentation system.
• Coordinates with referring hospital staff to obtain patient information for transfers.
• Coordinates with logistics team to fax demographic sheets to facilities.
• Places electronic bed orders for transfer team.
• Uses census data to project placement of incoming patients.
• Communicates high-priority patient movement and navigates placement barriers.
