Job summary

Methodist Health System is seeking a Patient Placement and Transfer Coordinator RN (PRN, night shift) to facilitate patient transfers within the PPTC. The role focuses on enabling physicians and patients to access MHS hospitals through standardized criteria and ongoing quality initiatives.

Requirements include a BSN, 3+ years healthcare experience (2+ years in acute hospital settings), and an active Texas RN license.

Qualifications

• Graduate from an accredited nursing program.
• BSN required, waiver may be granted for ADN with equivalent experience.
• 3+ years healthcare experience, minimum 2 years acute hospital experience, preferably Adult Critical Care and House Supervisor.
• Current RN license to practice in Texas.
• Ability to lift, push and pull with assistance (mechanical or co-worker) the weight of an average patient.
• Lifting 10 lbs-50 lbs maximum and occasional lifting.

Responsibilities

• Manages day to day PPTC calls. Facilitates placement and transfer with physicians, staff RNs and other departments while meeting state and federal regulations (COBRA/EMTALA).
• Ensures PPTC calls follow standards and protocols; identifies and reports quality issues to Medical Staff QA.
• Matches patient needs and system resources to optimize service value for patients, referral sources, and MHS facilities.
• Provides follow-up calls and communications with internal and external customers to meet stakeholders’ needs.
• Ensures compliance with PPTC workflows; receipt, tracking, documentation, formatting and timelines.
• Reports trends in PPTC workflows to management and identifies improvement opportunities.
• Uses problem-solving to determine best course of action for patient, MD, and hospital.
• Demonstrates high-level customer service with internal/external customers on every call.
