Job summary

Humana, Inc. is seeking a scheduling-oriented professional for CenterWell Home Health in Kentucky to manage patient schedules, coordinate staff coverage, and maintain clinical logs.

You will handle admissions timing, reassignment during absences, and ensure proper visit coverage with a focus on quality and patient care. The role requires strong communication, at least 1 year of home health experience, and familiarity with QA processes.

Qualifications

• Excellent communication and organizational skills; ability to interact with diverse groups.
• At least 1 year of home health experience.
• Prior packet review or quality improvement experience preferred.
• Coding certification is preferred.

Responsibilities

• Manage schedules for all patients and coordinate coverage during staff absences.
• Update agent unavailability in the scheduling system and reassign patients as needed.
• Initiate infection control forms and submit to HRD for uploading in the worker console.
• Serve as backup during lunch hours and busy periods, handling field calls and case conferences.
• Refer clinical questions to Branch Director and maintain hospitalization logs with coordination notes.
• Ensure staff are scheduled for skilled visits and add-ons as required by case conferences.
• Run scheduling reports (Agent Summary, Missed Visits) and prepare weekly schedules for the team.
