Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to schedule visits, coordinate with field staff, patients and physicians, and support day-to-day office operations. The role includes maintaining care coordination, managing staff schedules, and assisting with weekly case conferences.

The candidate should have strong communication and organizational skills, with at least 1 year of home health experience. Coding/QI experience and related certifications are preferred.

Responsibilities

• Schedule visits and coordinate with field staff, patients, physicians and other stakeholders.
• Manage schedules for all patients and reassign as needed when staff are unavailable.
• Update agent unavailability in the worker console and back up during peak times.
• Initiate infection control forms and route reports to appropriate teams.
• Prepare and review weekly schedules and productivity reports.
• Run scheduling reports and support case conferences as required.
