Job summary

Humana's CenterWell Home Health is seeking a Patient Services Coordinator to manage patient visit scheduling and coordinate care with field staff, patients, and physicians to ensure continuity of care. The role also supports day-to-day office and staff management and may involve clinical tasks when needed.

The ideal candidate has at least 1 year of home health experience, strong communication and organizational skills, and the ability to prioritize multiple tasks.

Qualifications

• Must possess excellent communication skills and the ability to interact with a diverse group of individuals.
• Strong organizational skills and the ability to manage and prioritize multiple assignments.
• At least 1 year of home health experience is required.
• Prior packet review / QI experience is preferred, and coding certification is preferred.

Responsibilities

• Schedule visits and coordinate with field staff, patients, and physicians to ensure care continuity.
• Manage patient schedules, reassign visits when agents are unavailable, and update worker console status.
• Initiate infection control forms and route to HRD for uploading in the worker console.
• Serve as back up during lunch hours and busy times, aiding with weekly case conferences.
• Maintain hospitalization logs and distribute coordination notes to staff; run scheduling reports.
