Job summary

Humana Inc is seeking a Patient Services Coordinator to schedule visits and coordinate care across field staff, patients, and physicians. This role also assists with day-to-day office and staff management in a home health environment.

The position requires excellent communication, strong organizational skills, and at least 1 year of home health experience. Coding certification is preferred and there is a clear emphasis on maintaining accurate scheduling and care coordination records.

Qualifications

• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Responsibilities

• Manages schedules for all patients and edits schedules for agents calling in sick.
• Initiates infection control forms and routes them to the appropriate department.
• Assists with weekly case conferences and field staff calls to ensure coordination of care.
• Maintains hospitalization and coordination logs and shares updates with staff.
• Schedules visits, add-ons, and various orders to ensure timely patient care.
• Runs scheduling reports and prepares weekly schedules for review and distribution.
• Supports clinical activities such as lab report handling and on-call rotation if required.
